[ Application for Disaster-Related Basic Income by Proxy ]
1. Minor"s (born after January 20, 2002) legal representative or lineal ascendant
2. If an applicant cannot submit an application due to unavoidable circumstances (e.g. elderly people, people with reduced mobility, persons staying in special types of facilities, etc.) his/her spouse, his/her lineal ascendant or descendant, his/her lineal ascendant or descendant’s spouse, his/her spouse’s lineal ascendant or descendant, his/her sibling*, a social welfare facility employee, etc. may apply for disaster-related basic income by proxy by submitting a document proving his/her relationship with the applicant.
※ (Example) If the representative is a social welfare facility employee, a copy of the applicant and mandator’s IDs, a written confirmation of admission and a certificate of employment of the proxy must be submitted.
* Scope of sibling’s application by proxy: Siblings of those who does not having any lineal ascendant or descendant, those who are in a sanitarium, a mental hospital, or an institution, or those living abroad
